Telehealth Access for Special and Vulnerable Populations: Challenges and Opportunities for Care
Telehealth is a vital component of the health center mission to minimize barriers to accessing services. As lawmakers consider the future of telehealth policy, there is a growing need to understand the value and efficacy of telehealth as a tool to deliver quality care and expand access to patients who face barriers to accessing in-person services. Preserving telehealth access and supporting improved, equity-focused implementation is a crucial imperative for health center leaders and policymakers. Health center leaders on telehealth implementation and subject matter experts will discuss using telehealth to reach underserved populations such as people with disabilities, substance use disorders, who are experiencing homelessness or living in areas in areas affected by disasters. Panelists from urban and rural health centers will address current telehealth policies and the implications for future directions, share their experiences offering telehealth services and discuss how to navigate a changing policy environment. Challenges and Emerging Policy Solutions for Adult Immunizations: How COVID-19 Has Impacted the Landscape of Vaccinations
The COVID-19 pandemic led to many new or updated policy changes around immunization processes and procedures. Health centers adopted and adapted their workflows based on COVID and are now extending those adaptations to other immunization practices. In this session, panelists will explore how they have navigated reimbursement and access challenges brought to light by the pandemic, describe learned lessons in stocking and providing vaccines, and share which newer policies they’ve taken advantage of to increase adult vaccination rates.

Transforming Experiences of Bias, Racism or Social Injustice into Systemic Change
Community Health Centers were born in the struggles against racism and injustice. And yet, systemic racism is still a central cause of many of the health disparities and inequities experienced by our patients and our communities. Health Centers around the country are prioritizing strategies to dismantle systemic and structural racism. Addressing Intimate Partner Violence, Human Trafficking, and Safety needs amongst CHC Patients through Policy and Partnerships - NTTAP FEATURED
Cross-sector partnerships and upstream policy efforts can be powerful strategies that health centers can leverage to support patients that are experiencing intimate partner violence (IPV), domestic violence (DV), and/or human trafficking (HT). While there is data demonstrating increasing rates across various groups and geographic locations, it is well-known that instances of IPV, DV, and HT are under-reported. This harsh reality coupled with increasing financial, housing, transportation, and employment challenges exacerbate the factors that contribute to individuals experiencing IPV, DV, and HT. More concerning is that individuals from structurally marginalized groups, such as non-white racial and ethnic groups and those that identify as LGBTQIA+, experience higher rates of IPV, DV, and HT. Health centers care for the most racially, ethnically, and linguistically diverse patient population along with individuals from HRSA defined special populations, such as LGBTQIA+, agricultural workers, and immigrants to name a few. Recognizing and responding to IPV and other forms of violence within HRSA-supported settings of care is the proponent behind HRSA’s 2023-2025 strategy to address IPV. This strategy includes strengthening infrastructure and workforce capacity to support IPV prevention and response services at health centers. This session will feature a panel of experts from national and federal agencies, health centers, and primary care associations. They will speak to their current efforts while highlighting the role of health centers and key partners to support individuals experiencing IPV and other forms of violence. More importantly, attendees will have an opportunity to hear from panelists on promising practices, challenges they’ve encountered, and lessons learned to date. The goal of the session is to equip attendees with the knowledge, resources, and ideas to expand current health center efforts on recognizing and responding to IPV and other forms of violence. Bright Spots and Lessons Learned from Leveraging Grassroots Organizing Principles to Mobilize Health Center Advocates
The health center movement is strongly rooted in community advocacy and organizing, with health center advocates adapting strategies when health center funding was either threatened or there was an opportunity to expand it. The strength and reach of health center advocates throughout history is what has resulted in the evolution of the health center program to its present state. Given the current political uncertainties and societal conditions, health centers can navigate these challenging realities by revisiting this history and build upon the strategies to ensure that health center funding remains a priority with elected officials at the local, state, and federal levels.
In January 2023, NACHC recommitted its advocacy efforts to the historic roots of the health center movement by leveraging grassroots organizing strategies to tap into the collective power of advocates at health centers, primary care associations, and beyond. This session will provide attendees with an opportunity to hear from health center advocates that have participated in NACHC’s Grassroots Organizing Training. They will share how they’ve applied principles and strategies from grassroots organizing within advocacy efforts at health centers and primary care associations. Finally, this session will provide attendees an opportunity to share their current strategies, successes, and challenges as they outline their legislative and advocacy priorities for the year ahead. This round table session will discuss how health centers and PCAs are supporting states in maintaining and reconnecting patients to Medicaid and Marketplace coverage. The conversation will also be forward-looking in how to maintain robust outreach and enrollment strategies beyond the current Medicaid unwinding and how to continue advocating for the O&E workforce and activities at Community Health Centers. Join health center, PCA, and NACHC colleagues and experts for an opportunity to discuss outreach and enrollment ideas and strategies. Community Health Center staff who manage outreach and enrollment programing and initiatives within their health center are strongly encouraged to attend this discussion. Health Center, Primary Care Association and Health Center Controlled Network staff who are also involved with Medicaid renewals are encouraged to attend. An Unlikely Source: Tapping into Local and State Department of Labor Funding to Support CHC Workforce Initiatives
Health Centers receive the majority of their funding, policy support, and programmatic direction through the U.S. Department of Health and Human Services (HHS). However, a small cadre of health centers and primary care associations have started applying for funding disbursed to state and local agencies.
This session will feature health centers and primary care associations that have learned how to access funds that are disbursed by state economic development boards and labor departments to support their workforce development initiatives. Advancing Behavioral Health: A Conversation with SAMHSA and Community Health Centers
The Bipartisan Safer Communities Act has provided various funding opportunities through the Substance Abuse and Mental Health Services Administration (SAMHSA) to help communities across the nation tackle the mental health crisis. Grant funding opportunities have been provided through mental health services and awareness trainings, youth mental health, the Crisis Care 988 lifeline and Certified Community Behavioral Health Clinics. Health centers have steadily expanded behavioral health services in medically underserved communities over the past decade to address this access gap. The holistic, culturally responsive health center approach reduces access barriers and improves patient health outcomes. UDS+ and Preparing Your Health Center for Patient-Level Reporting
The Uniform Data System (UDS) is an invaluable resource for understanding community health centers’ impact on patients and communities. This session will provide key updates on HRSA’s UDS Modernization Initiative, which will transition health centers from reporting aggregate data at the health center level to reporting disaggregated data at the patient level. UDS Patient-Level Submission, or UDS+, will enable health centers to identify factors that influence quality, health outcomes, and health disparities through analyses stratified by race, insurance status, and other important patient characteristics.
UDS+ reporting will begin with the 2024 UDS reports submitted to HRSA in February 2025. This session will inform health center leaders on the steps required to ensure their health center is prepared for patient-level reporting in February 2024. Topics will include: the rationale and regulatory requirements for UDS+, Health IT modernization and working with your HIT vendor, UDS+ testing and implementation, privacy and security, and staffing/resource considerations. PCOR Meets VBC: Improving Patient-centered Outcomes with Value Based Care
Bringing value-based care to a health center involves more than just rethinking billing systems and financial management. Value-based care requires rethinking care to bring patient-centered stakeholder input into all aspects of care. Yet patient-centered value is challenging to define because it varies for every individual. Where most traditional stakeholders define value in terms of clinical outcomes and cost, patients define value in broader, more far-reaching terms.
Implementing a VBC model requires rethinking the way patients and staff interact within the health care delivery system. After a brief presentation of existing research on the impacts of value-based care implementation on patients and staff, three health centers that have implemented value based care discuss their experience with patients, caregivers and staff. Impacts of implementing value-based care on the patient experience, including overall trust in the health care system, provider communication, care coordination, treatment comprehension, timeliness of care, quality of life and SDOH will be examined, as well as impacts on staff well-being, retention and recruitment, and health center operations as a whole. CMS Priorities on Medicaid Redeterminations, Open Enrollment, and Health Center Outreach and Enrollment
Last year in part of the Public Health Emergency unwinding, all states were required to review their Medicaid beneficiaries eligibility status – a process known as Medicaid Redeterminations (or renewals.). As a result of this process, millions of people have been disenrolled from Medicaid Coverage over the past year. This unwinding has overlapped with Open Enrollment in private insurance made available through Federal and State-Based Marketplaces. Health centers employed over 7,500 outreach and enrollment staff and provided nearly 4 million assists in 2022, which makes them uniquely positioned to support patients and community members seeking health insurance coverage. Although a health center board’s only direct employee is the Chief Executive Officer, a board plays a significant role in addressing workforce challenges and opportunities through its strategy, oversight, and policy roles. This session will highlight some of the workforce challenges and opportunities health centers are experiencing, and share tips for boards to address these through strategic planning, budget approval, policy approval, and other forms of oversight. Panelists will discuss real world examples of how boards have addressed workforce during strategy, budgeting, and policy conversations.
