Addressing Staff Core Competencies to Build Workforce Capacity through the Certificate of Excellence in Agricultural Worker Health (COE) Program - NTTAP FEATURED
Health centers have a commitment to provide culturally and linguistically appropriate care for agricultural workers and their families. However, they struggle to recruit qualified staff and train them to meet the unique needs and challenges of this population. NCFH’s Certificate of Excellence in Agricultural Worker Health (COE) Program provides a base of knowledge through core competencies to help healthcare teams better understand the complexities of this population. This self-paced, online program consists of seven e-learning modules with an interactive curriculum, valuable resources, and a broad range of topics. These modules can be uploaded to a learning management system to facilitate access to staff and can be incorporated into a health center’s workforce training plan. Presenters will provide an overview of the COE Program and describe how it can be used to increase knowledge about the agricultural worker population. Presenters will also provide a demonstration of the e-learning modules and review the core competencies addressed. Participants will have the opportunity to provide program feedback, identify topics for potential future modules, share strategies with peers on effective program implementation to build workforce capacity, and engage in discussions on the core competencies to enhance patient care and ultimately improve agricultural worker health outcomes.

Heat is a growing concern for health center patients and staff. Hot days are associated with a range of physical and mental health harms that include adverse pregnancy outcomes, increased emergency room visits or hospitalizations, and injuries. Populations that health centers serve like children, pregnant people, people with disabilities, people with mental health conditions, people experiencing cardiovascular diseases, and agricultural and outdoor workers experience greater susceptibility to heat.

The Coffee Break Project: A community-driven model supporting farmer/rancher mental health and emotional well-being
Economic and environmental conditions make farming and ranching a stressful occupation. While awareness of mental health issues has increased since COVID-19, attention to addressing the mental and emotional well-being of agricultural workers lags behind. The Coffee Break Project, an initiative developed by agricultural community members, is intended to raise awareness of stress and mental health concerns among farmers, ranchers, and agricultural workers and use the relational networks that exist in rural agricultural communities to address these issues. The Project sprang from the concerns of community members who regularly interacted with the agricultural world in their personal and professional lives about the growing number suicides among their neighbors—fellow farmers and ranchers in rural Colorado. The group’s efforts grew into a partnership between members of the area’s agricultural community and Valley-Wide Health Systems, Inc., the region’s integrated behavioral health care provider. Presenters will share the strategies and tools of the Coffee Break Project model and reveal how model components have been used to successfully create awareness of agricultural worker mental health issues and break down cultural barriers that frequently prevent farmers, ranchers, and their workers from reaching out for support.

The Forgotten Many: Improving Healthcare Access and Outcomes for Haitian Agricultural Workers - NTTAP FEATURED
In recent years, the United States has experienced a notable influx of Haitian migrants, driven by a desire for a better life amid political turmoil, poverty, natural disasters, and violence in their home country. This migration journey is fraught with challenges, exposing individuals to health concerns such as malnutrition, infectious diseases, injuries sustained during the voyage, and mental health challenges. Upon reaching the U.S., Haitian migrants encounter additional obstacles, including cultural and language barriers, limited healthcare access, inadequate support systems, and apprehension about seeking care due to immigration status. Haitian agricultural workers, a subset within this migrating population, frequently encounter oversight and additional disparities in healthcare. This session will outline strategies for health centers to effectively reduce and address health disparities among Haitian agricultural workers. Additionally, promising practices will be shared, focusing on building trust, enhancing health access, and improving outcomes, even when faced with limited resources.

Improving Access to Health Care and Benefits for Migrant and Seasonal Farm Workers - NTTAP FEATURED
Reproductive health refers to the condition of an individual’s reproductive system during all life stages and sexual well-being. It is influenced by numerous factors, including social and cultural norms, healthcare access, resource access, insurance access, and education level. Already, migrant, and seasonal farm workers (MSFW) have limited access to preventative screenings, family planning, perinatal care, benefits, and services. The resources for MSFW birthing individuals are scarce. In this session, we will discuss how to enhance perinatal healthcare for pregnant agricultural workers by addressing immediate and systemic challenges such as fear and mistrust and the importance of standardizing the approval and application support process to improve the timeliness of completion and mitigate provider bias about "eligibility" for disability. Drawing insights from a recent project, we employ a Qualitative and Quantitative approach to illuminate critical aspects of the disability claims process. In the Qualitative segment, we explore the lived experiences of 50 pregnant women through a detailed Patient Questionnaire, emphasizing comfort levels, comprehension, and timeliness of disability support. This exploration unveils nuanced insights into the emotions, challenges, and perceptions faced by pregnant agricultural workers during the disability claims journey. Simultaneously, our Quantitative component involves a retrospective analysis of International Classification of Diseases (ICD) codes for State Disability Insurance (SDI) and Paid Family Leave (PFL), targeting instances of denied benefits by the Employment Development Department (EDD) in 2022. Through this analysis, we aim to identify patterns, gaps, and areas for systemic improvement. By combining qualitative narratives with quantitative data, this session will offer a holistic understanding of challenges and opportunities in maternal healthcare for agricultural workers, emphasizing the patient experience and addressing access to resources. Additionally, it will explore the benefits of a Medical-Legal partnership as a strategy that empowers healthcare providers, health center staff, and advocates to address the specific needs of MSFW. Resources and Strategies to Address the Health Impacts of Climate Change for Health Centers - NTTAP FEATURED
Community Health Centers are at the front line of climate change, as it deeply affects the communities that they serve. Historically, low-income communities, communities of color, and agricultural communities are at a disproportionate risk of the health impacts of climate change, especially from extreme heat events. To protect patient health, organizations are partnering together to build resources and communication strategies focused on identifying the impacts of climate change on health, with a special emphasis on extreme heat. In this session, you will hear from the Migrant Clinicians Network, which will discuss heat-related illness symptoms and prevention strategies for agricultural workers and overview their resources including a clinician guide for heat-related illness and culturally contextual resources to educate patients about preventing and recognizing heat-related illness. Power of Community Health Workers: Transforming SDOH Screening and Improving Population Health - NTTAP FEATURED
Underserved communities, encompassing populations such as seasonal and agriculture workers, disproportionately bear the impact of social drivers of health needs (SDOH). Within these diverse communities, healthcare providers skillfully navigate the intricate web of social, institutional, and environmental factors that intricately shape health outcomes. The emergence of SDOH screening has become pivotal, serving as a powerful tool to unveil barriers to care and allowing health centers to fine-tune service delivery. Recipes for Success: Health Center Healthcare Provider Training Partnerships
It is well recognized that healthcare provider training location has an important role in determining where trainees practice after they complete their training. According to the authors of Milbank Memorial Fund’s June 2023 report Training the Primary Care Workforce to Deliver Team-Based Care in Underserved Areas: The Teaching Health Center Program, “Residents are more likely to…remain to practice in underserved settings after graduation if they have positive and high-quality community-based training experiences that both build their professional networks in underserved areas and encourage them to continue to practice in these communities.”
This session will provide participants with information on essential elements of training partnerships and curricula to prepare health center healthcare providers to provide care for agricultural workers. Content experts from SeaMar Community Health Centers as well as the Rural Residency and Teaching Health Center Planning and Development Technical Assistance Centers will share lessons learned building and sustaining partnerships for health center healthcare provider training. Indiana Regional Medical Center will also share their experience with the Keystone Health Agricultural Worker Program. The Wisconsin Collaborative for Rural GME will discuss key features of training curricula and resources specifically designed to prepare healthcare providers to provide care for agricultural workers. Value-Based Care Models for Vulnerable Communities: Insights from the Waianae Coast Comprehensive Health Center Study
This workshop delves into the pioneering study by Waianae Coast Comprehensive Health Center (WCCHC) on implementing an AI-enabled Value-Based Care (VBC) model in healthcare. Amid the challenges heightened by the COVID-19 pandemic, WCCHC, a FQHC in Hawaii, collaborated with Foresight Health Solutions to integrate Artificial Intelligence (AI) and Machine Learning (ML) technologies into healthcare delivery. Explore the methods used, including predictive risk scoring, prescriptive analytics, and the utilization of Natural Language Processing (NLP) in Electronic Medical Records (EMRs). WCCHC successfully met the expectations of value-based contracts with Hawaiian health plans, resulting in a net savings of approximately $3.5 million in the first year and a 16.4% reduction in baseline costs. The agricultural worker rights movement is strongly rooted in community advocacy and organizing and was the springboard for the health center movement. Health centers have been innovative in providing whole-person care to agricultural workers and their families and have built trusting relationships with them in a culturally and linguistically inclusive manner. Given the current political and societal conditions, health centers can navigate current realities by revisiting this history and build upon the strategies that have shaped agricultural worker health programs. It is critical that health centers are able to nurture a network of diverse, intergenerational community advocates that can strategize and act on health center priorities for agricultural workers and their families. This session will explore how to create and sustain a culture of community advocacy and organizing that is rooted in relational power, and how it can deepen the commitment and engagement of staff, board members, and patients in advocacy. The key strategies of storytelling and the act of invitation will be reviewed along with community advocacy and organizing principles.
