Alert management solutions such as those provided by FDB (First Databank), can help proactively manage medication CDS, ameliorating many of the negative consequences of poorly targeted alerts.
While the benefits are relatively clear, building the internal case for adoption of an alert management solutions within a provider organization can be very challenging, due to competing initiatives, lack of alignment with central organizational goals, de-prioritization of alerting as an institutional priority, and challenges with establishing hard financial and clinical ROI.
In this focus group, Steve Miller, MD, MBE, a clinical informatician and pediatric gastroenterologist, will conduct a roundtable discussion on best practices for crafting of an internal business case to drive adoption of an alert management solution in the current era of constrained resources and competing priorities. Using Technology to Support Dementia Care
As the prevalence of dementia continues to rise globally, leveraging technological innovations becomes increasingly important for enhancing quality of life for people living with dementia and their caregivers. In this session, panelists will describe four unique and complementary research projects, focused on improving dementia care. Dr. Powell will describe findings from a NIH-funded study aimed at reducing avoidable hospitalization of nursing home residents with dementia. This research team is building a new ontology based on the Age-Friendly Health Systems 4M (what Matters, Mobility, Mentation, Medications) framework to mine text messages exchanged among a multidisciplinary team as they make decisions regarding hospital transfer. Dr. Alexander will describe work from a NIH-funded study examining the impact of nursing home leadership, care environments and health information technology on outcomes of residents with Alzheimer’s Disease and Related Dementias (ADRD). Dr. Knoo Lee will describe a study funded by the Alzheimer’s Association aimed to integrate the use of smart home sensors with artificial intelligence to monitor and detect ADRD-specific daily activities among older adults, with the goal of early symptom detection and personalized support. Finally, Ms. Ashley Woods, a PhD candidate from the University of Missouri, will describe her work on a NIH-funded supplement examining how pain assessment and management is represented in text messages exchanged among a multidisciplinary team when communicating about patients with ADRD. Panelists will describe advancements, challenges, and best practices for using technology to address the complex needs of this vulnerable population and will engage in dialogue with each other and audience members.

Developing a Clinical Informatics Course for Medical Students

There is a growing need to educate medical students about the world of clinical informatics. During this workshop, attendees will learn how physician Clinical Informaticists have developed Clinical Informatics courses in asynchronous online, synchronous online, and hybrid formats. Attendees will learn how to navigate curriculum requirements to develop a successful course proposal. Attendees will understand how to develop a syllabus (including elements such as student contact hours), recruit students, and house the course in a learning management system. Additionally, the workshop will focus on curriculum development for adult, graduate-level learners. Organizational Issues and Informatics: Translating Theory into Practice
Focused attention on both technical and organizational aspects of health information technology is required for support successful implementation and ongoing use1. Organizational aspects include workflow, technology acceptance and sociotechnical factors, among others2-4. Fields such as Organizational Theory and Change Management offer a substantial foundation of evidence-based guidance on how to navigate the human and organizational aspects of technology-based change5-9. Support is needed to translate theory and concepts from multiple fields into clinical informatics practice in a way that’s accessible for organizations. The goal of this workshop is to draw from evidence in related fields to bridge the gap between theory and practice. Clinical Informatics Fellowship Curriculum Design Workshop
Clinical Informatics fellowship (CIF) is a relatively new specialty with training and curriculum that evolved from the original core content published in 2009 to the more recent Clinical Informatics Subspecialty (CIS) Delineation of Practice(DoP) published in 2019. The current 60 ACGME-accredited CIF programs have considerable variability in training, skill, and expertise of their Program Directors (PD) and faculty. Being a relatively new and non-clinical specialty the opportunities and available training for PDs and faculty to acquire the requisite knowledge and skills for creating and implementing a curriculum, evaluating fellows and programs have been minimal. With a field that was and is rapidly evolving, the original core content outline was insufficient for developing competencies on which fellows could be taught and evaluated. Furthermore, this outline did not encompass all informatics-related expertise required by graduating CI fellows in post-fellowship practice. This led the Clinical Informatics Program Directors (CIPD) Curriculum Subcommittee to incorporate the 2019 DoP guidelines into the existing structure for training fellows. In this workshop, the authors (a subgroup of the CIPD curriculum committee), will use a small group format to guide the attendees in creating a fellowship rotation block diagram mapped to specific sub competencies, creating learning objectives for each rotation which are mapped to appropriate milestones and configuring specific fellow assessment and program evaluation processes. The final product of this workshop will be the basis for future standardization of fellow training experience across various programs.

Digital Equity for Health Equity: State of the Science and Art of Practice
Digital health inequities emerged among marginalized communities during the rapid expansion of telehealth services during the COVID-19 pandemic. Digital health equity has emerged as a national and international priority. This panel will take on this new and important topic by bringing together the perspectives of health policy, health informatics research, community organizations, and technology development. The session will offer a landscape of initiatives that aim to catalyze digital health equity covering a broad range of technologies from broadband to digital health and generative AI. The speakers will address the historical foundations contributing to digital inequities, introduce emerging frameworks for assessing and planning digital health equity programs, and share pioneering examples of real-world digital health equity in action. Discussion among the panelists and with the audience will explore implications of digital health equity for healthcare stakeholders, health informatics researchers and practitioners, policy makers, and the public. This session addresses the critical topic of digital health equity that is timely given the emergent funding opportunities and national investments to provide broadband internet for all households and to support digital access for healthcare, education, and employment. Exploring Epic’s De-identified Cosmos Database: Real-world examples using maternal-infant data.

Future of Standards-Based Interoperability in Cardiovascular Health: The CardX Domain of the HL7 CodeX FHIR Accelerator
This panel presentation discusses the future of standards-based interoperability in cardiovascular health, focusing on the CardX domain of the HL7 CodeX FHIR Accelerator. Cardiovascular disease is the leading cause of death globally, and progress in care and research is slowing. Panelists from the American College of Cardiologists, the University of Nebraska Medical Center’s Center for Intelligent Health Care, Duke University, and the MITRE Corporation will discuss the critical interoperability challenges facing cardiovascular care today and the potential of HL7 FHIR-based solutions to address these challenges. Attendees will learn about CodeX, the clinical FHIR Accelerator and CardX, the cardiovascular domain of CodeX. They will also understand how to participate in CodeX and CardX use cases, particularly the development of mCARD, a common set of data elements to support cardiovascular data interoperability. Implementation & Outcomes from a Clinical Informatics Consult Service focused on EHR Improvements
Whereas clinicians may be able to easily highlight opportunities ripe for improvement, they often lack technical knowledge of electronic health record (EHR) capabilities to propose solutions. We developed the Clinical Informatics consult service to translate EHR optimization requests into achievable solutions that address provider and patient needs. Information is gathered and refined by a clinical informatics fellow and attending then shared with a multidisciplinary team of physician and nursing informaticists who evaluate and discuss consults weekly in a manner akin to a tumor board. Each consult resulted in a formal recommendation and reasoning to the requestor. In the first 30 months of operation, our Clinical Informatics consult service processed 305 consults with 146 (47.9%) recommended for EHR change, 83 (27.2%) representing existing EHR functionality, 22 (7.2%) representing upcoming EHR functionality, and 54 (17.7%) recommended as not technically feasible. Consults were requested by 48 different clinical services, including both medical and surgical subspecialties as well as a variety of non-patient care departments. Physician Informaticists coded the consult requests into types. EHR change was the most the common recommendation for all types (enhancement, efficiency, communication, safety, and compliance) except research. EHR optimization has been shown to decrease physician burn out, and our consult service helped to reduce the friction in initiating EHR changes. In almost three years of operation, our consult service has received several hundred consults from a wide variety of stakeholders, grown year on year, and demonstrated value by matching EHR-related end user needs to appropriate informatics solutions.

Promises and Pitfalls: Ensuring Equity in Generative Artificial Intelligence for Primary Care
The presenters will participate in a panel discussion focused on how emerging generative AI solutions can promote equitable patient care and potentially widen the digital divide in primary care. The technologies we plan to cover include ambient listening documentation, patient portal draft replies, generative summaries for patient education, AI language interpretation and translation, and chatbots. Panelists represent internal medicine and pediatrics primary care informaticians with experience in the intersection of health equity and technology. This will be an engaging and open dialogue on how informaticians can assess and balance the impact of generative AI on delivering equitable care to patients. Without early discussions on health equity, generative AI risks leaving marginalized populations behind. The generative AI space is evolving rapidly and session content will be updated based on relevance to the clinical informatics community.

Team-based Primary Care Tools: Balancing Patient Access with Clinician Burnout
Workforce shortages amongst physicians, nurses, and staff have led to patient care delays and increased workplace burnout. The CONCERN Study: Stages of an Interdisciplinary AI Enabled and Applied Clinical Informatics Intervention Study
Our team has worked for over a decade developing the CONCERN (Communicating Narrative Concerns Entered by RNs) tool that leverages nurses’ expert and knowledge-driven behaviors within the electronic health record (EHR) to support early prediction of decompensation for hospitalized patients. The CONCERN artificial intelligence (AI) driven predictive model identifies nurse documentation patterns by analyzing EHR metadata (i.e., data about data) that are consistent with nurses’ concerns. In other words, the presence of documentation beyond minimum requirements is information that the nurse likely perceived that an event or observation was clinically significant enough to record. We have validated the CONCERN tool in a multi-site randomized control trial and are implementing it more widely. This novel analytical approach can shift how we understand and leverage clinical observational skills and clinician entered data within a patient’s chart for clinical decision support. This panel will provide an overview of the research and implementation steps to bring an AI-based predictive model to the clinical setting, including stakeholder engagement, rigorous evaluation, end-user workflow and burden considerations, bias evaluations and mitigation, and cost-benefit analyses.

AI in Clinical Care: Hype or Helpful
In the past couple of years AI has become a major consideration in all industries. In healthcare AI is already being touted as a major disruptor. In practice health systems have found that the value of AI products may not be as clear cut as the hype would suggest. Clinicians on FHIR: How FHIR Enables Interoperability
HL7 created Fast Healthcare Interoperability Resources (FHIR) as a next generation standard to address clinical interoperability. Now FHIR has evolved from a standard in trial use to a requirement under US federal rules as an API for patient access to their data in EHR systems as well as their claims data in payer systems. Additionally, the NIH has indicated their intent to support the expansion of FHIR capability for clinical and translational research. Clinicians on FHIR workshop is continually evolving to educate clinicians about the latest HL7 FHIR and provide feedback to the HL7 FHIR team regarding the clinical viability and usability of the FHIR standard. This pre-symposium workshop will bring the Clinicians on FHIR activity to a broader clinical audience attending the AMIA Fall Symposium. The workshop is designed to educate attendees about HL7 FHIR and tools available to access, review, and provide feedback to the HL7 FHIR team regarding the evolving HL7 FHIR standard. It is also intended to make the audience aware of the potential of FHIR for innovation in their organizations. The faculty will provide lectures describing HL7 FHIR history, background, and fundamental principles. Examples of applications using the SMART on FHIR platform will also be discussed. The new federal requirements for FHIR APIs in EHR systems will be outlined. After this initial overview of FHIR, the attendees will be guided through using online tools to examine HL7 FHIR Resources and build basic common FHIR Profiles for use in implementation guides for specific use cases.

Workshop: Knowledge Management at Scale

Different types of organizations build increasingly large amounts of knowledge assets. Whether or not this activity is overtly recognized and proactively managed, the organization is ultimately responsible for the accuracy and availability of the knowledge they produce. Inconsistent, incomplete, outdated, and inaccessible knowledge assets contribute to operational costs and create quality and safety risks. Such costs and risks are greater when knowledge management activities are reactive and ad-hoc, lack a formal content review and maintenance process, and are not aligned with business goals and strategies. This workshop will survey basic and advanced knowledge management topics, such as definitions of knowledge, data, and metadata; the knowledge engineering lifecycle; effective governance; best practices to manage dependencies between knowledge artifacts as they evolve; and efficient processes to guarantee structural and semantic integrity of knowledge assets. The second half of the workshop will describe the experiences, challenges, lessons learned and solutions implemented by a large-scale operational commercial knowledge management program for curating oncological treatment plans. Because this domain is semantically complex, voluminous, and constantly changing, it represents an ideal use case for both beginners and advanced informatics professionals to learn about knowledge management.

CMIO/CHIO Leadership Workshop

Clinical informatics includes over 2,000 board certified physicians from 54 accredited CI Fellowship Programs. For 12 years, the authors have presented a CMIO workshop at the AMIA symposium and for several years at the Clinical Informatics Conference. Many of the attendees at these workshops have attended previously, attesting to faculties’ effort to keep the material current. The attendees range from seasoned CMIOs of large systems to those who are just beginning their applied clinical informatics careers. The 2024 CIC Workshop would be a combination of personal growth opportunities as well as the chance to debate current challenges facing clinical informatics leaders. Each year new topics are selected so that the content remains fresh, and the 2024 workshop would focus on navigating artificial intelligence, negotiating for more resources, building the value proposition for clinical informatics, and analyzing your personal leadership style. The faculty will leverage didactic as well as small and large group exercises to provide participants the opportunity to be actively involved during the workshop and to get their individual questions answered. Workshop will also include self-reflective and analytical exercises so that participants leave the workshop with concrete skills that they can then apply at their home institution. In summary, the workshop would be an opportunity for clinical informatics leaders at all levels of experience to develop new concrete leadership skills while also connecting with each other so that our field continues to thrive.

While interruptive alerts may improve health quality and safety due to their perceived benefits that users notice and respond promptly, they come with drawbacks, including an immediate increase in cognitive load and task interruption.There is a high risk to provider burnout through the cumulative impact of interruptive alerts. Thus, there is a need to consider less intrusive options before employing them.In this panel we will describe examples from multiple institutions within the Pediatric Clinical Decision Support (CDS) Collaborative where creative, non-interruptive CDS solutions were more effective than interruptive (i.e., pop up) alerts.

AI-Powered Solutions for Patient Flow Optimization
Hospital operators often face challenges in making critical patient flow decisions due to the lack of centralized data and reliance on heuristics. Tampa General Hospital (TGH) and Cleveland Clinic (CCF) addressed these issues by using Palantir Foundry to build and deploy customized applications powered by integrated data, codified business logic, and machine learning/large language model-based predictive analytics. TGH’s patient placement tool offers AI-generated predictions of clinically appropriate beds based on patient-level clinical characteristics and hospital-wide capacity metrics. This has led to a 30% decrease in OR/PACU hold times, and Bed Planners use these recommendations to reduce off-service placements. CCF's Hospital 360 module optimizes patient flow across multiple hospitals within the enterprise by leveraging AI to streamline decision-making based on staff availability and bed demand. This has led to a 10.5% increase in transfer patient acceptance. Palantir Foundry has transformed how TGH and CCF use integrated data and predictive analytics to improve patient throughput. The implementation of these advanced tools has led to significant improvements in patient care quality and safety, as well as operational efficiency.

Applying the AMIA Inclusive Language Content and Style Guidelines to Clinical Informatics Practice
Inclusive language in clinical and scholarly communication is important to promote equity in health care. Unfortunately, existing clinical research shows that stigmatizing language, linguistic bias, and negative patient descriptors are common in electronic health records (EHRs). Secondary uses of narrative language documented in EHRs and open notes may be subject to data and algorithmic biases. Clinical informatics applications based on natural language may reflect these biases. Clinical informaticians must be aware of these language biases to mitigate unintended consequences, such as perpetuating health care disparities. New technological applications based on unstructured data must also be subjected to thoughtful implementation and rigorous evaluations; unfortunately, there are no existing examples of mitigation methods for text-based algorithms and generative artificial intelligence. Finally, clinical informaticians who publish scholarly work would benefit from communicating their work using inclusive language. Inclusive language is crucial to operationalizing clinical informatics innovations in an equitable manner, and conducting ethical and rigorous research while promoting accuracy, fairness, and inclusivity. In Spring 2023, the AMIA DEI Committee released its first edition of the AMIA Board of Directors-approved Inclusive Language and Context Style Guidelines (ILCSG) to provide four guiding principles (plurality, precision, transparency, and destigmatization). Workshop goals include familiarizing clinical informatics practitioners and scholars with the ILCSG, sharing best practices for their application in communications, and further use the opportunity for participants to brainstorm novel ways to apply ILCSG principles to their own informatics practice and also to share feedback as members of the AMIA community to improve the ILCSG.

Cancer is Different: Exploring the Impact of Immediate Access to Cancer-Related Test Results on Patients and Clinicians
The Final Rule of the 21st Century Cures Act mandates the immediate availability of electronic health data, including all test results upon patient request, prompting concern about releasing highly sensitive or lifechanging results without prior review and consultation by a clinician. The intent to improve information sharing offers significant benefits to both patients and clinicians. While a growing body of research demonstrates that patients are generally accepting of health data transparency via the patient portal, questions remain about whether this remains true in the setting of life-changing test results such as may occur in cancer medicine. The panelists will describe and discuss findings from the current research on open results, reflect on anecdotal experiences, and offer suggestions for best practices around patient counseling for open cancer results. Audience members will engage in this robust discussion about organizational experiences, opportunities to improve patient wellbeing, and ongoing initiatives to improve Open Results.

Designing telehealth interventions for the unique needs of marginalized older adults
Marginalized older adults face numerous barriers to telehealth use despite having the greatest health needs. Therefore, understanding barriers and opportunities for rethinking the design and implementation of telehealth interventions to better meet their needs is critical. In this panel that represents racial, gender, and professional diversity we will provide an overview of the key barriers to telehealth use among marginalized older adults at the patient, interpersonal (caregiver), structural, and organizational levels. We will describe multiple interventions the panelists have implemented to address these barriers and present interventions conducted in different geographic regions across the U.S. with different populations of older adults, describing both common themes and unique factors.

EHRs Under Examination: The Role of Clinical Informatics Experts in Legal Proceedings
As clinical information systems become deeply embedded in every care process, they increasingly play a role in legal proceedings. For example, malpractice cases may hinge on what documentation means or who had access to what information and when – information that may be present in the EHR and/or the audit log. Contract disputes between hospitals and information systems vendors may arise, as well as intellectual property issues where parties disagree about whether a particular clinical information system feature infringes on a patent.
Expert witnesses play a crucial role in legal proceedings by providing specialized knowledge and opinions on complex subjects that are beyond the understanding of the average person, judge, or jury. Medical expert witnesses are routinely called in trials but, increasingly, the expertise of clinical informaticians is needed. Clinical informatics experts may be asked to interpret EHR audit logs, assist with onsite inspections of EHR-related evidence, provide opinions about the appropriate design and use of clinical information systems, and provide information on the history of the field and prior art in intellectual property disputes.
In this panel, we will review the roles and qualifications of expert witnesses, share the experience of several clinical informatics expert witnesses, and discuss future directions for clinical informatics expert work.

One Size Does Not Fit All: Perspectives on Patient Portals, Whole-Person Health and Co-design for Increasing Consumer Engagement in Health
With the increasing emphasis on value-based and person-centered care, there is a growing momentum to increase patient engagement with electronic health information via the use of consumer-facing digital health tools such as patient portals that enable online access to medical records and apps that collect patient-reported outcomes. This session will provide multiple lenses into how patient-facing digital health tools are being used, and have the potential to be used, to increase patient engagement in their health. We will highlight how large national datasets can be used to understand aggregate trends in use of digital health tools across subgroups and over time, focusing specifically on patient portal use. We will then describe how questionnaires can be used to dig deeper into individuals’ current and desired use of patient portals. We will provide an example of how mobile health (mHealth) apps can be used to collect whole-person health data, including social determinants of health data. We will describe an approach to digital health technology development, termed co-design, that can better ensure the technologies meet the needs of patients. The panelists will also address current gaps in techquity—technology developed and implemented with health equity at the forefront— and possible approaches to address them.

Visual Analytics 101: A Hands-On Introduction to Data Visualization

This three-hour instructional workshop is designed to address the growing need for AMIA members, particularly clinicians, to acquire fundamental knowledge and skills surrounding the principles of Visual Analytics. The workshop, endorsed by the Visual Analytics Working Group, aims to give participants hands-on experience with data visualization and teach them the principles from the ground up. The half-day workshop will offer collaborative, team-based design activities and interactive visualization development sessions. Workshop leaders with experience in healthcare data visualization will guide participants through the session. Optimizing Teams to Maximize Informatics-focused Innovation, Equity, and Impact: Lessons from Health Systems, Industry, and the Public Sector
This interactive workshop will employ brief presentations on leadership approaches to cultivate collaboration, informatics-supported innovation, and equity, accompanied by discussion and small group breakouts for hands-on ideation and problem-solving activities. While it is geared toward early career faculty/professionals in healthcare settings, the workshop is open to a general audience, including clinicians, educators, researchers, developers, and policymakers. Successful problem-solving in healthcare requires multidisciplinary expertise – both formal training and practical experience – grounded in understanding the specific needs of different interested parties. Formal clinical research and informatics training emphasizes mastery of domain-specific knowledge and technical skills, often leaving the development of other critical capabilities such as communications, team building, management, and systems thinking as extracurriculars. Yet these capabilities are integral to driving and leading healthcare transformation at scale across various settings. The importance of interdisciplinary learning, cross-sector collaboration, and partnerships are underscored by a growing body of evidence demonstrating the value of diverse teams on productivity and outcomes. These collaborations are critically important as new multidisciplinary analytic methods and technology platforms provide greater opportunities to innovate. Applying insights from team science and collaboration literature, industry best practices, and AHRQ’s core competencies for advancing learning health systems, the workshop faculty will describe the emerging skills, methods, and informatics and health services research literature that support more equitable, innovative, and scalable solutions in healthcare. Participants will explore how to combine applied informatics with innovative strategies in the context of diverse collaborations and understand how multidisciplinary and multi-sector teams can partner for more effective problem-solving.

Prompt Engineering 101: Evidence-Based Practices for your Health System

With a majority of health systems experimenting with deploying generative artificial intelligence (AI) tools such as OpenAI’s GPT-4, it is crucial for healthcare providers to become proficient in applying these technologies effectively. A key component of this proficiency is prompt engineering—the art and science of crafting inputs to elicit accurate and relevant outputs from large language models (LLMs). Despite its importance, there is a notable deficiency in healthcare-specific educational resources on prompt engineering, making it a potentially daunting task for the average clinical end-user. This interactive workshop is designed to bridge this educational gap. Through a combination of didactics interwoven with small group application activities, this session will cover various prompting techniques such as zero-shot, few-shot, Chain of Thought (CoT), Least to Most (LtM), and Retrieval Augmented Generation (RAG). It will focus on up-to-date, evidenced-based prompting techniques and their application to real healthcare cases, facilitating a deeper and more practical understanding of prompt engineering. By the end of this session, attendees will have the ability to construct effective prompts for healthcare use-cases and a working proficiency in the evaluation of LLM performance for medical tasks, enhancing their ability to integrate generative AI tools into patient care.
