Development and Application of Social Determinants of Health Data

Integrating social risk data into the EHR and implementing social care interventions are critical to mitigate the impact of social risk factors, predict health care utilization and health outcomes. Developing innovative methodologies while leveraging data and digital approaches to identifying health-related social needs will drive the safety, efficiency, effectiveness, and sustainability of healthcare delivered. Substantial national efforts are underway to advance SDOH interoperability and enable the collection and use of social risk data. Notably, the Gravity Project is a national public collaborative that develops consensus-based data standards to improve use and sharing of social risk data. To this end the Gravity Project’s terminology workstream has developed multiple value sets for exchanging data across 17 domains of social risk. Informed by this body of work a group of experts facilitated a conceptual exploration of social risk data at the NIWG-endorsed workshop at the 2022 Annual Symposium. Building on that, this collaborative workshop will: 1) explore the current state-of-the-science related to social risk data, 2) identify immediate priority data elements to enhance the sharing and use of social risk data, 3) advance the conceptualization and application of these elements, and 4) chart a path forward to promote the use of social risk data beyond the meeting. The goal is to engage participants in an applied discussion with fellow colleagues to co-create a vision and strategy for social risk data within informatics. This workshop aims to move from conceptualization to application, with plans to continue building towards social care interventions in future workshops.
