W20: Examining Implications and Use of Social Determinants of Health Data

The purpose of this workshop is to discuss and gain knowledge related to the use and inclusion of Social Determinants of Health (SDOH) data to support and improve individual, community and population health. The workshop will explore the inclusion, interoperability and use of SDOH data in electronic health records and digital health tools to empower and to partner with patients, families and communities and interprofessional colleagues/care teams for whole person care. Addressing social determinants of health (SDOH) are important for improving health and reducing health inequities. SDOH data refers to conditions in which people live, learn, work, and play and affect a wide range of health risks and outcomes.1–3 A key to addressing improving health and reducing health inequities is access to and use of accurate SDOH data. Furthering the use of SDOH data across platforms requires consistent use of data standards to advance interoperability. There have been national recommendations to create data standards for representing SDOH.
