The 2020 ACMI Symposium focused on the nation’s public health informatics ecosystem needed to support essential public health services, particularly in the context of COVID-19: case reporting, communication and dissemination of evidence-based guidance, real-time situational awareness, crisis care management, etc. Members of ACMI and invited guests met to better understand ongoing challenges and questions posed by public health leaders, share information about existing initiatives, discuss relevant innovations and strategies, and debate priorities for local, state, and national policymakers to consider when creating and implementing programs. In this session, individuals involved in planning the event will share the recommendations and discuss how AMIA can support rebuilding the nation’s public health informatics infrastructure going forward.

W12: Analysis of Human Interactive Behavior for Improving Patient Safety and Clinician Burnout

Most clinical environments resemble a paradigmatic complex system with its dynamic and interactive collaborative work, non-linear and interdependent activities, and uncertainty. Addition of new organizational and systemic interventions, such as health IT, can cause considerable cascading effects in the clinical processes, workflow, and consequently, on throughput and efficiency. A 2011 IOM report called for a sociotechnical approach for designing and incorporating health IT in clinical settings1. One of the critical aspects of a sociotechnical approach is to understand the progression and evolution of human interactions within a sociotechnical context. In this instructional workshop, we will discuss a set of convergent methodologies for analyzing human interactive behavior both with technology and with other humans or artifacts. These methodologies can help in capturing underlying patterns of human interactive behavior, and provide a mechanism to develop integrative, longitudinal metrics (e.g., metrics related to performance, or errors) for clinical activities for sustained interactive episodes that evolve over time. Such analyses can also provide significant input for reducing clinician burnout and improving patient safety outcomes through the design of safer and more efficient health IT. We have successfully organized this instructional workshop at AMIA in the last few years. We propose this new workshop with a greater focus on capturing and analysis of human interactive behavior to improve health IT design and ultimately patient safety and clinician burnout. This sixth workshop will also target an audience with intermediate and advanced levels of expertise, instead of the preliminary level that it was previously designed for.

W03: Functional Status Informatics in Health and Healthcare: The Third Workshop on AI for Function, Disability, and Health

Functional status describes the lived experience of health, and understanding the factors that affect functioning is key to delivering patient-centered healthcare. Global aging and the growing prevalence of multi-morbidity and disability are making understanding functioning outside the clinic equally important to diagnosis and treatment inside the clinic. However, information on functional status has proven difficult to capture and utilize within existing information systems and informatics paradigms, leaving a space ripe for technological innovation. The AI4Function workshop series is a venue for researchers across data science and artificial intelligence, rehabilitation science, and clinical informatics to discuss new ways to collect and utilize functional status information within healthcare delivery, public health, and social well-being. Functional status is complex and multidimensional, including information on activity performance, social role participation, and environmental and personal factors that affect work, well-being, and quality of life. The AI4Function workshop series reflects both theoretical and practical aspects of this complexity, including research on tools and methods for functional measurement, methodologies for analyzing multimodal function data, and informatics-driven interventions for monitoring and optimizing functional status. The 2022 edition of the workshop will focus on fostering collaborations and identifying key challenges and directions for functional status informatics research in AMIA. The workshop will include: (1) a panel discussion on data resources for research on functional status; (2) a poster session for informatics researchers studying functional status to share recent or upcoming work; and (3) a discussion session to identify next steps for growing functional status informatics in the AMIA community.

W23: Lessons from Industry Science: Scaling Biomedical Informatics Research at the Speed of Technology Evolution

Description of Workshop Conducting biomedical informatics research as a healthcare technology company poses specific challenges. Research activities to support product design, development, and evaluation must be conducted rapidly, to meet product delivery deadlines and to keep pace with rapid evolution of technology and the corresponding changes in business priorities. To enhance scientific integrity, research can be conducted in collaborations with clients or academic partners, and such collaborations require careful protection of confidential health data and intellectual property. Scientific publications by technology companies are often scrutinized in the peer review process, and thus the underlying science must have a foundation of exceptionally rigorous and reproducible methods. In this 3-hour instructional workshop, we will present an overview of lessons learned and best practices for conducting scientifically rigorous research at a rapid, pragmatic, industry pace. This tutorial should be of interest to informaticians who are planning to collaborate with industry, wish to transition from an academic to an industry career path, or would like to accelerate their research productivity in any environment. The first 2 hours will be an interactive didactic session. A one hour working session will allow participants to get direct feedback from panelists on accelerating their research efforts and discuss real-world experience when conducting research during the COVID-19 pandemic.

CMS (Center for Medicare and Medicaid Services) defines health equity as the attainment of the highest level of health for all people, where everyone has a fair and just opportunity to attain their optimal health regardless of race, ethnicity, disability, sexual orientation, gender identity, socioeconomic status, geography, preferred language, or other factors that affect access to to care and health outcomes. These factors or SDOH (Social Determinants of Health) are widely recognized as important predictors in clinical care, and patients with social risk factors have worse outcomes and increased costs. During this session, we will explore the benefits, barriers, considerations, and strategies to developing programs that screen for, collect and intervene on SDOH factors in your community, while leveraging data analytics and the EHR. We will explore how the EHR can be leveraged to collect, analyze, and act on this new kind of data, and what should organizations be doing now to build a strong health IT foundation for a successful approach.

W09: A Workshop to Build a Research Agenda for Justice Informatics
SOLD OUT
Dominant and mainstream discourse in the US has come to recognize justice as integral to population and individual health. These critical conversations provide a unique opportunity to advocate for justice’s implementation in informatics and allow for articulation of common goals for justice-informed practice. An important first step is collaboratively formalizing definitions, principles, and actions to organize and support methodical future research. Through this workshop we will bring together an interdisciplinary group of stakeholders in biomedical informatics to formalize a working definition of justice informatics (JI), discuss an agenda for future informatics work, and take first strides towards compiling resources to aid JI. Before the workshop, we will crowd-source a preliminary definition of JI from the broader informatics community, and gather information about how bias and oppression impacts daily informatics work. In the first phase of the workshop, we will refine this preliminary definition. In the second phase of the workshop, attendees will work together to articulate, ideate, and develop JI-oriented, actionable agendas for areas of informatics practice, e.g., scholarship (research methods, citational justice), leadership and development (curriculum development, mentorship), and policy. During the third phase of the workshop, participants will collaborate to document key takeaways, compile resources, and develop a toolkit to guide and inform engagement of future research and resource development. These conversations will carry forward through the creation of a community network and the publication of a paper.

S84: Panel - Advancing Health and Social Service Information Exchange: Cutting Edge Examples, Lessons Learned, and What More Needs to be Learned
Currently, there is an upswell in the development and implementation of health information technologies designed to facilitate bidirectional information exchange between healthcare organizations and community-based organizations (CBO), enabling social care referrals from healthcare organizations and post-referral follow-up with CBOs. Vendor-derived community resource referral platforms and homegrown solutions are rapidly being implemented, though there is limited evidence available regarding best practices for implementation, ethical considerations, and effects on social and health outcomes. Our panel will present some of the latest research related to health and social service information exchange from the perspective of several stakeholder groups: statewide government payer-implemented programs, developers of a community-based cross-sector data exchange and collaboration technology, and CBOs. Findings will highlight current research gaps and opportunities to inform the next generation of policy and practice interventions to improve information exchange between healthcare and community organizations.

S62: Panel - Bioethics perspectives on the development of urgently needed informatics solutions to address rising maternal morbidity and mortality
Informatics interventions for maternal health are ethically complex but critically needed given rising maternal morbidity and mortality. In this panel, we will explore and synthesize the ethical principles and values that surround the development and implementation of informatics interventions for maternal health, especially those that use patients’ personal health data. In addition to presenting primary data from multiple studies with key stakeholders, we will also discuss potential policies that may best uphold the core values and guiding ethical principles within the rapidly changing landscape of maternal health-focused informatics interventions. By the end of this panel, participants will be able to identify ethical principles and values relevant to informatics interventions for maternal health and discuss strategies for upholding ethical values in the context of case studies presented by the panelists.

S44: Panel - Building the Evidence Base for Automating Public Health Surveillance through a Nationwide Medication Safety Surveillance Use Case
Inpatient hypoglycemia is one of the most common adverse drug events (ADEs) occurring in U.S. hospitals (1). Hypoglycemia in hospitalized patients can be severe and is associated with longer hospital stays, higher medical costs, and increased morbidity and mortality (2-5). Varying rates of hypoglycemia and glycemic management practices across U.S. hospitals suggest opportunities to improve medication-related hypoglycemia measurement in a meaningful and standardized way (6,7). The Centers for Disease Control and Prevention (CDC)’s National Healthcare Safety Network (NHSN) Glycemic Control, Hypoglycemia module is the first NHSN module intended for inpatient ADE surveillance; it combines an open-source Fast Healthcare Interoperability Resources® (FHIR) application (NHSNLink) and FHIR-based approach to using electronic health records (EHRs) as source systems for electronically reporting linked inpatient blood glucose and medication administration data to NHSN via industry-standard, vendor-neutral electronic messages. This fast response surveillance framework enables convenient data aggregation and rapid development, which significantly facilitates the deployment of surveillance measures.

S104: Panel - Can Data Science Move Medical Device Surveillance Forward? Highlights of Solutions and Remaining Challenges in Real-World Evidence Generation
This panel seeks to describe current solutions and future potential of data science when applied to medical device surveillance. While observational data science has exploded in the last decade, numerous challenges remain, some intrinsic to the observational data setting, some resulting from interoperability challenges, and some unique to the field of medical devices. For example, the lack of uniform device identification capture is often accentuated by the complexities of aligning device sub-components, materials, or manufacturing techniques for cross-device evidence generation. In addition, rapid market uptake in device use demands novel methods and tools for separation of learning effects from device intrinsic safety signals. We will bring together experts from the FDA, medical device partnerships MDEpiNet and NEST, and academic researchers to describe and highlight the current state, challenges faced, and opportunities for data science to play a part in future solutions, with examples in vascular and cardiology devices among others.

S94: Panel - Designing Informatics Solutions for Older Adults with Cognitive Impairment: Responding to the Challenge
Older adults experiencing cognitive decline are often excluded from research studies because of concerns about their ability to engage, consent, or provide valuable insights in a timely manner. Similarly, they are often left out of design and testing efforts for new technologies conceptualized to improve quality of life. Funding organizations such as the National Institute of Aging and the Alzheimer’s Association have recognized the need for designing tools for older adults experiencing cognitive decline. Yet, research on how to design for older adults with cognitive limitations and dementia remains difficult because of misperceptions about their ability to participate and confusion about consent, caregiver burden, and access issues. The purpose of this panel is to showcase person-centered informatics research to facilitate ongoing engagement of older adults with cognitive impairment and their families.

S12: Panel - Evidence in Clinical Practice Records: Gathering and Analysis of Real-World Data for Trials and Outcome Studies
Investigators use increasingly more often data recorded during routine care to generate scientific evidence. New models use real-world evidence (RWE) approaches to supplement or substitute data in control arms of clinical trials with matched EHR records or with data gathered prospectively from patient care. RWE designs allow robust comparators where controls are not possible for ethical, feasibility or cost reasons and can speed up rare disease trials. Effectiveness studies using “clinical trial emulation” use RWE to determine benefit to patients. We will characterize core concepts of RWE research, including site selection, aggregation, and data transformation into research-grade datasets. Panelists will discuss study design considerations and lessons learned from the perspectives of a pharmacology sponsor, a contract research organization, technology provider and a healthcare organization. This fast-evolving segment of research and a timely perspective on best practices is relevant to informaticists and researchers in private, clinical and academic domains

S24: Panel - Making a Case for Adoption of ICD-11 Morbidity Reporting in the U.S.: Multiple Perspectives
The International Classification of Diseases Eleventh Revision (ICD-11) is now in effect(1). According to the WHO, 35 countries are using ICD-11 and some of the implemented uses are primary care, patient safety, dermatology, and clinical documentation.(2) While countries are moving forward, the US is in very early discussions regarding adoption and implementation. A panel consisting of one of the primary developers of ICD-11 and a clinician, a researcher, and a health information professional will share their perspectives on why ICD-11 should be adopted in the US before the next decade. Objectives or outcomes that participants can expect to achieve: 1) Assess the reasons ICD-11 is superior to ICD-10; 2) Recognize what can be gained from ICD-11 implementation; and 3) Determine the significance of ICD-11 adoption for morbidity reporting in the US.

S04: Panel - Need FAIR Evidence? Use CEDAR to Discover and Retrieve Research Findings
Accessing evidence-based resources such as systematic reviews and practice recommendations often requires time-consuming, manual searching of multiple repositories and websites. The Agency for Healthcare Research and Quality (AHRQ) aims to make its evidence repositories more FAIR (findable, accessible, interoperable, and reusable) through an application programming interface (API) called CEPI Evidence Discovery and Retrieval (CEDAR). CEPI is AHRQ’s Center for Evidence and Practice Improvement, which is the home of evidence repositories that support the Evidence-based Practice Centers, the U.S. Preventive Services Task Force, and CDS Connect. Resources include systematic reviews, preventive services recommendations, clinical decision support artifacts, and others. This panel will present the CEDAR project from the perspectives of AHRQ, the engineering team at MITRE, and the American Academy of Family Physicians, which piloted the CEDAR prototype, and will share generalizable approaches to harmonizing metadata, using standards, and incorporating CEDAR output into products aimed at improving clinical decision-making.

S36: Panel - Putting the user back in user-centered design: Strategies for incorporating patient goals and values throughout the design of decision aids
User-centered design approaches, while growing in popularity, vary widely across healthcare research intervention development. An important consideration when determining the success of an intervention created with user-centered design is whether the “patient mandate”, or the patient’s needs and requests, are met. A barrier to creating patient-centered designs is understanding patient needs even if they are not articulated by the patient, or directly contradict their requests. Moreover, those conducting user-centered design work frequently do not have adequate guidance or resources to understand how to carry out this “patient mandate.” In this panel, we will provide an overview of a cognitive engineering framework for evaluating patient needs, a synthesis of design implications for researchers on user-centered design, and a range of use cases of user-centered design for decision aids.

As consumers (aka patients) & healthcare providers; we all know how broken healthcare end to end healthcare journeys have become. Over the last decade cloud computing, artificial intelligence, machine learning and other automation technologies have had a profound impact on rationalizing end-to-end consumer journeys. Over the last decade, innovators at the bleeding edge of informatics have been applying these technologies to solve real world and research model problems. FHIR® data fabrics, CQL engines, BPM+ Health workflows and other health standards technologies have changed the art of the possible for real time, useful, usable decision support. W07: Co-designing Health Informatics Solutions with Patients and Community Members: A Collaborative Workshop

Co-design and co-production (to which we refer in this abstract collectively as co-design) are similar concepts rooted in the principle that solutions are more relevant to the problem of interest and better able to produce the intended outcome if the users and others affected are engaged in the definition, design, and implementation. In consumer health informatics, it is particularly important that the patient and community member perspectives and voice are prominent throughout the co-design process. Evidence suggests the use of co-design in consumer health informatics is a nascent, fragmented, and rapidly evolving area of research, with little consensus on terminology and methods. Co-design encompasses a variety of frameworks, philosophies, methods and approaches. In this six-hour collaborative workshop on co-design for consumer health informatics challenges we aim to enhance understanding of the principles and key methods. We aim to build connections among AMIA members interested in co-designing with patients and community members for mutual learning and resource sharing. We will provide interdisciplinary perspectives on co-design from researchers, practitioners, patients, and community partners. Presentations will include historical and philosophical foundations of co-design and participatory research, an introduction to co-design approaches and methods in consumer health informatics, and real-world and research co-design examples in a diverse group of settings and communities. The interactive portions of the workshop will include facilitated, hands-on opportunities to apply skills to proposed design challenges, discussions about needs and additional resources for AMIA members, and an exploration of priorities and opportunities for further research and engagement with community members.
